Please include any contact information you may have for the practitioner to assist us in more easily contacting them. Feel free to include any information regarding their practice that you feel is relevant to help us in evaluating them for inclusion.

* PRIVACY NOTICE:  We do not make unsoliticed phone calls to physicians. To ensure the integrity of the referral process and to verify for doctors that they were truly referred to us, we do normally provide the name of the individual who referred them to us. We do not give out any other information, ever.

     If you would like your referral or recommendation to be ANONYMOUS, please say so clearly in your message to us. We do respect all such requests and take your privacy seriously.
